REGISTRATION REQUIREMENTS (NEW STUDENTS)

 

Only a parent /legal guardian can register a student.  Any questions about legal guardianship will be referred to the Central Office.  A notarized statement does not establish guardianship.  Guardianship goes through the court process and is granted by a judge.     

 

1.      Certified Copy of Birth Certificate or a money order receipt showing that the birth certificate has been ordered.

 

2.      Two proofs of residence:

      Any parent who can not produce two proofs of residence must be referred to the

      Central Office.   They will need to bring a notarized Statement to the Central Office.     

      The parent will be given a permission letter to return to the school.  This permission letter

      will be one proof of residence.  You will have 30 days to provide South Side Lower with 

      the second proof. All cases will be verified by home visit. 

      Letters from Central Office will be for the 2016-2017 school year.

    

3.      Immunization Report:

The primary source for the Immunization is the Pearl River County Health Department.  An Immunization Report from a physician is acceptable as long as it is on the State form.

Two proofs of residence:

      Any parent who can not produce two proofs of residence must be referred to  

      Central Office.  They will need to bring a notarized Statement to the Central Office.     

      The parent will be given a permission letter to return to the school.  This permission   

      letter will be one proof of residence.  You will have 30 days to provide South Side

      Lower with the second proof. All cases will be verified by home visit.  Letters from  

      Central Office will be for the 2017-2018school year.
